Preparing for your visit

After you have made your appointment, our office will send you a patient information packet or you can download them here. Please fill out as completely as possible. Feel free to bring any past medical records and x-rays that you may wish your doctor to review. Be sure to bring a list of all medications you are currently taking. IF you belong to an HMO, please make sure your primary care physician has sent a referral to our office. Please arrive 15 minutes early for you appointment in order to register. You will be asked to fill out necessary papers for our office to obtain your personal and insurance information.

What to expect during your office visit

On your first visit, your doctor will listen to the description of your condition and ask you specific questions regarding your complaint. Your complete medical history will be reviewed along with any pertinent medical records. A complete physical examination will be performed. In some cases, treatment plan will be recommended based on this information. Additional lab work, x-rays, or scans may be necessary to develop a plan of treatment. Completion of this evaluation may require a follow-up appointment.

Professional Fees and Insurance

Payment is due at the time services are rendered. If your insurance is an HMO plan, obtaining a referral is your responsibility. If a referral is not obtained prior to your appointment, this may result in your appointment being rescheduled. AOA accepts most insurance plans. It is your responsibility to make sure that we are participating with your insurance company. We accept Visa and Mastercard for your convenience. We are happy to file your insurance, however, you will be responsible for any portion of your bill not covered by your insurance plan. Make sure you bring your insurance identification card to your appointment. If you do not have insurance, full payment is required at the time of service.
